Job Description

POSITION OVERVIEW:   

Assists the Director of Nursing with the day-to-day operations, functions, and activities of the nursing department, ensuring compliance with all federal, state, and local standards, guidelines, and regulations that govern the facility. 

ESSENTIAL FUNCTIONS: 

Leadership and Staff Management 

  1. Support and develop a cohesive nursing team by leading, motivating, and supporting staff in daily operations. 
  2. Promote a positive and professional image through leadership activities aligned with the organization’s mission, fostering cooperation and teamwork. 
  3. Ensure sufficient nursing personnel and/or Registered Nurses are available for each shift to maintain quality care; communicate staffing needs to the Scheduling Coordinator and Director of Nursing. 
  4. Supervise nursing staff and initiate constructive discipline and performance evaluations in accordance with facility policies. 
  5. Oversee staff education and in-service programs to maintain professional competency. 
  6. Monitor, track, and administer Tuberculosis screenings (TSTs) and annual influenza vaccinations to ensure regulatory compliance. 
  7. Ensure completion of corrective actions identified in medical record audits and consultant reports. 

Resident Care Oversight  

  1. Complete routine rounds on all shifts to assess residents and ensure their needs are met according to standards of care, resident requests, and resident rights. 
  2. Manage all resident care programs, including Incident & Accident reporting, Infection Control, Wound Care, Coumadin, QAPI, Safety, Incontinence, Restorative Care, Labs, Pharmacy Requisitions, TB monitoring, and related programs. 
  3. Ensure all admissions and re-admissions are processed in compliance with federal, state, local, and facility policies; review Admission Checklists for completeness and accuracy. 
  4. Review unit 24-hour reports daily and supervise/coordinate nursing services and care plan interventions. 
  5. Review resident medical records and nurses’ notes to ensure documentation is accurate, complete, and reflective of resident care and responses. 
  6. Address clinical issues, participate in rounds, and ensure follow-up by Unit Managers. 
  7. Participate in the development and review of individualized care plans, ensuring that goals, interventions, and responsibilities are clearly defined and communicated to all personnel. 
  8. Serve as a point of contact for residents and families, addressing concerns, complaints, or grievances, and escalating when appropriate. 
  9. Spot-check CNA documentation for accuracy and completeness, providing one-on-one education as needed. 

Clinical Oversight and Compliance 

  1. Ensure all licensed nurses comply with procedures for medication administration, storage, and control, including narcotics. 
  2. Observe medication passes, treatments, and documentation to ensure compliance with policies and regulations. 
  3. Provide direct nursing support on units and assist Unit Managers, House Supervisors, and Charge Nurses with resident assessments and notifications. 
  4. Provide direct care coverage as needed to address staffing shortages due to absences, vacations, or other challenges. 
  5. Participate in on-call rotations as directed. 
  6. Perform administrative duties, including medical reports, forms, evaluations, charting, and studies as assigned. 
  7. Inspect nursing service areas (stations, medication rooms, supply rooms) to ensure cleanliness, organization, safety, and compliance with standards.

Regulatory Compliance and Safety 

  1. Attend mandatory in-services and facility or departmental meetings. 
  2. Ensure compliance with OSHA, Infection Control, and Universal Precautions, maintaining a safe work environment. 
  3. Maintain HIPAA compliance and safeguard resident confidentiality. 
  4. Assist with oversight and updates to the Nursing Policy and Procedure Manual in coordination with the Director of Nursing. 
  5. Monitor PPD testing for new hires (including 2-step process), annual staff screenings, and report results to Employee Relations for tracking. 
  6. Manage medication prior authorizations and act as a liaison with Pharmacy. 
  7. Assist with oversight of the QAPI committee and maintain 24-hour reports, completing audits and in-services as assigned. 
  8. Provide clinical oversight for best practice compliance, including observation of care and documentation of staff performance. 

ADDITIONAL FUNCTIONS:  

  1. Provide staff with information regarding patient rights, facility policies, and regulatory updates (federal, state, OSHA, CDC). 
  2. Participate in recruitment, interviews, and hiring of nursing personnel as directed. 
  3. Assist with monthly chart audits, MARs, and TARs. 
  4. Assist with or complete performance counseling of staff when necessary. 
  5. Attend and participate in charge nurse and CNA meetings, continuing education programs, committees, and workgroups as assigned. 
  6. Oversee Relias learning system with assistance from the Scheduling Coordinator. 
  7. Other duties as assigned. 

Qualifications

KNOWLEDGE, SKILLS, AND ABILITIES:  

  1. Proficient in Microsoft Office applications, including Word, Excel, and Outlook. 
  2. Previous experience with electronic medical records (EMR) software is preferred. 
  3. Ability to remain calm, composed, and professional in complex or challenging situations. 
  4. Strong organizational and time-management skills, with the ability to prioritize tasks, make decisions, delegate responsibilities, and manage multiple objectives and deadlines effectively. 
  5. Excellent verbal and written communication skills, with the ability to interact professionally with residents, patients, staff, families, vendors, and other stakeholders both inside and outside the facility. 
  6. Demonstrates patience, empathy, and a positive attitude toward the elderly population, with a genuine commitment to serving the community. 

EDUCATION AND EXPERIENCE:  

Current, unrestricted Registered Nurse (RN) license issued by the Commonwealth of Virginia Department of Health Professions, or enrollment in an accredited RN program. Current CPR certification (must be obtained within 30 days of hire if not already certified). 

Three to five (3–5) years of nursing experience, preferably in a long-term care or senior living setting. Minimum of six (6) months of supervisory experience preferred 

PHYSICAL REQUIREMENTS:  

  1. Ability to stand, walk, sit, stoop, bend, and reach for extended periods. 
  2. Ability to lift up to 20 pounds frequently (up to 66% of the time spent in performance of duties) and up to 35 pounds occasionally (up to 33% of the time spent in performance of duties). 
  3. Manual dexterity that allows grasping, carrying and holding equipment and supplies or for resident care as needed to perform tasks. 
  4. Ability to see, hear, and verbally communicate in the performance of duties. 

SPECIAL REQUIREMENTS:  

Requires a satisfactory general background check, including state and local criminal history checks, as well as a satisfactory drug screening.
