Key Responsibilities

CHAP Accreditation — Medicare Census Build

• Screen incoming referrals for Medicare eligibility and homebound status, selecting appropriate patients for the CHAP survey census
• Conduct initial assessments and complete OASIS-E documentation accurately for up to 10 concurrent Medicare patients — these patient records form the primary audit material for the CHAP site visit
• Provide or coordinate skilled nursing visits through patient discharge, ensuring clinical documentation meets Medicare Conditions of Participation
• Maintain survey-ready chart quality throughout the census build period

CHAP Accreditation — Policy, Compliance & Quality

• Conduct a comprehensive gap analysis of existing policies and procedures against CHAP Community Health Accreditation Partner standards
• Build and maintain the full agency policy and procedure manual to CHAP standards
• Establish and operate the QAPI (Quality Assessment and Performance Improvement) program, including quarterly meetings, outcome tracking, data collection, and corrective action planning
• Develop and maintain infection control, emergency preparedness, and patient rights documentation
• Plan and lead internal mock survey in advance of the CHAP site visit
• Serve as primary agency contact for the CHAP application process and site visit coordination
• Post-accreditation: maintain ongoing compliance calendar and manage the annual standards review cycle

Assistant Director of Nursing — Ongoing Medicaid Operations

• Review and co-sign nursing notes for Medicaid patients, verifying clinical accuracy, visit frequency compliance, and payer documentation requirements
• Assist the Director of Nursing with prior authorization requests and renewals, with particular focus on the complex authorization cycles for pediatric PDN cases
• Assist with plans of care (485s) — ensuring clinical accuracy, managing physician signature follow-up, and ensuring timely processing
• Conduct 60-day supervisory field visits alongside or in place of the DON — including direct observation of field nurses, patient status review, and care plan updates
• Provide clinical and documentation guidance to field nursing staff as needed

Long-Term Track — Administrator Development

• Shadow the owner/administrator on AHCA compliance, survey preparation, and operational decisions
• Gradually assume increasing responsibility for day-to-day agency operations
• Obtain or confirm AHCA home health administrator license eligibility under Florida Statute 400.474
• Transition formally to the agency administrator role on a mutually agreed timeline, targeted within 12–24 months

Qualifications

Required

• Active Florida Registered Nurse (RN) license in good standing
• Minimum 2–3 years of home health field experience
• Demonstrated OASIS-E competency — ability to complete OASIS assessments accurately (OASIS certification preferred)
• Working knowledge of Medicare Conditions of Participation and home health documentation standards
• Valid Florida driver’s license and reliable transportation for field visits

Strongly Preferred

• Prior experience with CHAP, ACHC, or Joint Commission accreditation processes
• Experience with Medicaid and/or MMA prior authorization for home health or PDN services
• Background in QAPI program development or clinical compliance
• Prior supervisory, DON, or clinical coordinator experience in a home health setting
