Position Highlights:The Primary Care Provider - Advanced Practice Clinician (APC) functions as an independent primary care provider responsible for the comprehensive, longitudinal clinical and administrative coordination of care for an assigned panel of patients. This role is intended for experienced APCs who have demonstrated readiness to independently manage a full primary care panel within a coordinated, value‑based care environment. Providers practice in a collaborative team model and assume accountability for clinical decision‑making, care coordination, and outcomes over time. You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Applicants must meet all of the following:Licensure & EducationNurse Practitioner (NP): Master's degree (MSN) and national certification (ANCC or AANP) or Physician Assistant (PA): Master's degree and certification by NCCPAActive, unrestricted Massachusetts NP or PA license (or ability to obtain prior to start)Active DEA and MCSR (or ability to obtain prior to start)Current BLS certificationExperience & ScopeMinimum of three (3) years of experience independently managing a full primary care patient panelExperience includes responsibility for:Preventive careLongitudinal careChronic disease managementCare coordination and follow‑upExperience caring for adult patients ages 65 and olderDemonstrated experience practicing in an ambulatory primary care setting, not limited to episodic, urgent, or specialty‑only carePrior responsibility for inbox management, test results follow‑up, patient messaging, and referral coordinationApplicants who do not currently meet these required qualifications may be eligible for future transition into a fully paneled role. Preferred Qualifications:Experience practicing in a value‑based, population health, or coordinated care modelExperience managing medically complex adult patients with multiple chronic conditionsPrior practice within a team‑based primary care model utilizing interdisciplinary supportExperience working within standardized clinical pathways or quality initiativesEpic EHR experience The salary range for this role is $109,500 to $164,000 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
