Position Overview: Wellness Coach

Hours: Full-Time AM or PM shift available. Follows a 4-2 rotation schedule.

The Wellness Coach is an unlicensed behavioral health support role responsible for promoting a safe,

structured, and recovery-oriented therapeutic environment. This position

reflects a proactive, engagement-focused, and recovery-centered approach to resident support. The Wellness

Coach focuses on observation, therapeutic engagement, emotional support, and activity facilitation rather than

skilled nursing care or hands-on ADL assistance. Services are delivered under the direction of the Program

Director and with mentorship from Counselors.

Essential Duties and Responsibilities

Program Delivery and Resident Services:

• Facilitate scheduled therapeutic and skill-building groups as assigned.

• Follow approved BH curriculum and structured group format.

• Implement behavioral skill-building interventions including:

o Coping skills

o Emotional regulation

o Social skills training

o Independent living skills

o Prevocational skill reinforcement

o Addiction recovery support (when applicable)

• Maintain group standards and adhere to the daily program schedule.

• Adapt groups to meet residents’ cognitive, psychiatric, and physical needs.

• Communicates with residents to check on physical, emotional and spiritual needs.

• Assists in establishment and maintenance of program objectives by observing and reporting unusual

resident behavior to the charge nurse.

• Encourage consistent participation to support weekly engagement goals.

• Reinforce individualized behavior support plans during group and individual interactions.

• Promote generalization of skills across settings (room, unit, dining area, activities).

• Provide coaching and redirection in alignment with trauma-informed principles.

• Model respectful communication and recovery-oriented language.

Documentation & Compliance

• Complete timely and accurate documentation of:

o Daily group notes

o Individual progress notes (as assigned)

o Attendance and engagement tracking forms

• Document observable behaviors, skill use, and measurable outcomes.

• Maintain documentation consistent with the facility’s Golden Thread framework.

• Report concerns, incidents, or behavioral changes to licensed staff.

Team Collaboration

• Attend interdisciplinary meetings as assigned.

• Communicate resident progress and concerns to Program Director and nursing staff.

• Participate in supervision and ongoing training.

• Support audit readiness and quality assurance activities.Crisis Prevention & Safety

• Participate in crisis prevention and de-escalation as trained.

• Reinforce coping skills during emotional dysregulation.

• Maintain a safe and structured therapeutic environment.

Knowledge, Skills and Abilities

• Understanding of trauma-informed and recovery-oriented care.

• Ability to facilitate structured groups with confidence and consistency.

• Ability to maintain professional boundaries.

• Strong documentation skills.

Education and Experience

• High School Diploma or GED required.

• Associate’s or Bachelor’s degree in Psychology, Social Work, Human Services, or related field

preferred.

• Experience working with individuals with serious mental illness, co-occurring disorders, or behavioral

challenges preferred.
